News from Wayne Memorial Hospital

An enthusiastic group of Wayne Memorial Hospital (WMH) and Wayne Memorial Community Health Centers (WMCHCS) administration, board and medical staff along with the Pike County Commissioners are shown cutting the ribbon at the official opening of the Pike Dental Center and Pike Behavioral Health Services offices located in the Pike Family Health Center, 750 Route 739, Lords Valley.

Plus, Medicare patients of Wayne Memorial Community Health Centers’ (WMCHC) primary care practices now have an advocate specifically tasked with managing continuity of care.  Carollee Keating, RN, was recently appointed as the Case Management Registered Nurse as part of WMCHC’s membership with Keystone Accountable Care Organization (ACO) in April 2013.

According to the health centers’ administration, the addition of this position allows WMCHC to more closely evaluate care utilizing a team approach by holding multi-disciplinary staff meetings and discussing best patient outcomes.

In her new role, Keating will follow up with patients to keep appointments, conduct medication reviews and arrange additional services for those coping with complicated situations.  She looks forward to “being a member of a collaborative team and helping patients understand that properly maintaining their healthcare objectives will lead to better quality of life.”

Keating is a graduate of Luzerne County Community College.  She has been a registered nurse for more than 17 years.  Her practical experience includes Telemetry, Neonatal Intensive Care, Hemodialysis and Wound Care.

The Centers of Medicare and Medicaid Services support physicians and hospitals who form ACOs as a means of providing better, coordinated care across all of the different places their patients receive services.  WMCHC patients may review a list of Frequently Asked Questions about ACOs by visiting the home page of www.wmh.org/WMCHC or calling 570-253-8207.
